Membership Cancellation Request

1. Your membership cancellation request must be received by the 15th of the month before the next billing cycle. If you give notice after the 15th of the month a $75 fee will be charged.

2. If you decide to reactivate your membership in the future (and we hope you do) remember membership rates in effect at the time of reactivation will be applicable.  Membership rates are subject to change.

3. If you would like to put your membership on hold instead of canceling, you can submit a Membership Hold Request. 

4. If your membership is charged and you decide to leave you must tell the Studio Manager/Owner by the 5th of the month. A $125 fee will be assessed.
–If any classes were attended during this time, $25 per class attended will be charged.
–A check will be sent for the remainder of the balance.

REMEMBER: Agreements are auto renewed unless the MEMBERSHIP CANCELLATION FORM is submitted to us by the 15th of the month.
